Computer Frequently Asked Questions (FAQ) -- How to Request ITS Access for Employees, Partners and Volunteers

The following forms need to be completed to gain access to ITS computer resources. These requirements apply across the board to anyone needing access to USDA computer resources and email accounts. This includes permanent employees, temporary employees expected to work more than 30 days, contractors, volunteers, and partners (Conservation District and RC&D employees).

Agency employees should have background investigation documentation filed within their Official Personnel Folder (OPF).

All Conservation District and other partner employees who connect their computers to the USDA network must also have background investigations. This includes employees who do not have Common Computing Environment (CCE) logins and use the connection only to access the internet.

The employee is a new NRCS employee - what forms do they need?

The some of following documents requires Adobe Acrobat or MS Word, as noted.

Supervisors:  Please print and provide the following mandatory items to the new employee:

*Request for ITS Access (PDF; 44 KB)  (Supervisor completes this form)
*Self-Certification Form (Computer Security Awareness Training) (PDF; 53 KB)
*SF 85, Questionnaire for Non-Sensitive Positions (takes you to another webpage where forms can be found)
*USDA-NRCS Computer User Security Agreement (PDF; 26 KB)
* SF 87, Fingerprint card (two sets required) -- Available in the Human Resources Office.
 OF-306, Declaration for Federal Employment   -- Ensure form is on file with HR.
 OF-612 or Resumé,  Optional Application for Federal Employment -- Ensure form is on file with HR.

The new employee is a volunteer or contractor or affiliated with the SWCD or RC&D, - what forms do they need?

Required Forms for volunteers & contractors, SWCD Personnel, and RC&D Employees:
The following  documents requires Adobe Acrobat.

Supervisors:  Please print and provide the following mandatory items to the individual:

*SF 85, Questionnaire for Non-Sensitive Positions (takes you to another webpage where forms can be found)
* USDA-NRCS Computer User Security Agreement (PDF; 26 KB)
* SF-87, Fingerprint card (two sets required) -- Available from Human Resources Office.  (See note below)
*OF-306, Declaration for Federal Employment  (takes you to another webpage where forms can be found) -- an OPM requirement, even non-Federal employees such as partners & contractors)
*OF-612,  Optional Application for Federal Employment (takes you to another webpage where forms can be found) -- an OPM requirement, even non-Federal employees such as partners & contractors).  You may substitute this with your resumé.  Make sure to provide a physical location address for each employer within the last five years.
